The wording of a certificate of recognition can range from very formal to casual.

The wording of a certificate of recognition can be formal and classic, or it can have a more informal, contemporary style. When deciding the degree of formality to use, consider what is most appropriate for the group or organization that gives the award, and for the particular achievement that prompts the recognition.
Instructions
1. State the name of your company or organization on the first line of the certificate after the top margin.
2. Use the words "Certificate of Recognition" on the next line.
3. (Optional) Choose the words for the third line, before the person's name. (Whether you use this step depends on the wording you prefer in step 5.) Suggested phrases are:
"is hereby granted to" (more formal)
"is hereby awarded to"
"Presented to" (less formal)
"Awarded to"
4. State the full name of the person receiving the certificate.
5. Choose the words for the fifth line. If you used one of the suggested phrases, or a similar phrase, in step 3, some suggested text is:
"in honor of your achievement in..." (more formal)
"in honor of your completion of..."
"in honor of your excellence in..."
"for outstanding performance in..." (less formal)
"for completion of..."
If you chose not to use any text in step 3, some suggested phrases are:
"We hereby present you with this certificate of recognition" (more formal)
"Has successfully completed..." (less formal)
6. Include the date that the certificate will be presented:
"On the nn Day of MM in the Year YYYY" (more formal)
"On this xx Day of month, year"
"MM DD, YYYY" (less formal)
